Chronic Kidney Disease Program

According to the National Kidney Foundation, 26 million Americans have chronic kidney disease (CKD) and millions of others have increased risk.

Those considered to be at increased risk include people with hypertension, diabetes, or a family history of kidney disease.


Early detection and treatment can help keep CKD from progressing. As the disease progresses wastes build to high levels in the blood; complications such as high blood pressure, anemia (low blood count), weak bones, and nerve damage can occur and increase the risk of heart and blood vessel disease. Chronic kidney disease may eventually lead to kidney failure, requiring dialysis or kidney transplant.


CKD PROGRAM AT NEPHROLOGY ASSOCIATES

The CKD program at Nephrology Associates is designed to help you keep your kidneys as healthy as possible for as long as possible. You will meet with a team of health care providers who specialize in CKD. This team will work with you to set up a long-term treatment plan which will include:

  • Prevention of further damage to the kidneys.
  • Management of problems which often occur with kidney damage such as anemia, bone disease, high blood pressure, dietary problems.
  • Planning for dialysis or transplantation if needed.


Patient Experience

  • Chronic Care Management (CCM) - Receive a personal care coordinator who will work with you to create your care plan and provide coordination of care between your Primary Care Provider, Specialists, and Pharmacy. Your coordinator will also assist with all of your medication needs.
  • Remote Patient Monitoring (RPM)– Receive blood pressure monitor to take your blood pressure from home. Daily readings will be sent automatically to your care coordinator to manage.
  • Transitional Care Management (TCM) – Receive a care coordinator to assist with all needs and schedule follow up appointments with your Nephrologist after hospitalization.
  • Education for Slowing Your Kidney Disease – Our CKD Nurse Practitioner will help you understand your stage of kidney disease and how to slow the progression. 
  • Nutritional Therapy – Our Registered Dietician provides nutrition therapy, including Renal and Diabetic diet education, complete with grocery lists, meal plans and recipes.
